Contest Rules

  1. Participation in this contest is restricted to persons residing in the state of New Jersey, U.S.A. and members of American Fitness Associates Inc.'s "Image Planet World Class Sports & Health Club for Men and Women" and "Image 2000 Health Spa for Women" facilities who reside outside of New Jersey, U.S.A.
  2. A winner will be drawn at 5:00 p.m. EST on the last Friday of every month. All questionnaires must be completed by midnight EST on the last Thursday of the month to be eligible for the drawing that month. All questionnaires completed after midnight on the last Thursday of the month will be entered in the drawing for the following month. Only one questionnaire per person. Only questionnaires completed in their entirety will be eligible for the drawing.
  3. The winner will be notified by email. Prizes must be claimed within 30 days of the drawing.
  4. No substitutions of prizes will be allowed. Prizes may not be redeemed for cash or other consideration.
  5. Image Planet World Class Sports & Health Club for Men and Women and its agents and affiliates may use a winner's name without compensation in advertising and otherwise. Image Planet World Class Sports & Health Club for Men and Women and its agents and affiliates are released from any and all liabilities arising out of the preparation or release of any information relating to anyone's participation in the contest.
  6. Employees and agents of Image Planet World Class Sports & Health Club for Men and Women, its subsidiaries and affiliates, and members of their families and their households are not eligible.
